Quite what the defence were up to it's hard to tell. West could almost have an excuse for not ruffing on the second round of hearts, but what was East doing? If he returns heart9 then West is bound to ruff. The club return guarantees one down. The only opening lead to break 4spade for sure is ace and another heart. A club return and heart continuation lead to two down. This defence is certainly possible if 4spade has been reached after South has decided to bid 2heart on the first round instead of a negative double.
