Simon Stokes asked whether East led a club against 6NT. On a diamond or spade lead declarer can run five diamonds and three spades. West is strip-squeezed and must either discard four clubs, in which case declarer can set up a second club winner, or a heart which allows declarer a third heart trick. All quite plausible.

The play on a heart is more in the realms of double-dummy..
