Instead of leading a low spade from dummy at trick four, declarer should surely have led an honour and, if South discards, declarer ditches his losing club. West ruffs the club return and crosses to dummy with a trump to lead the spade winner. South must ruff, but now declarer over-ruffs, draws all the remaining trumps by crossing to dummy and cashes the last spade for a heart discard.

If South ruffs the third round of spades, declarer over-ruffs and establishes the fifth spade without further problems.
