What's happened here?

What's happened here is that West has lost his marbles and East is trying to obscure his partner's error. My correspondent confessed that he crossed to dummy's diamondA, cashed another diamond throwing his spade loser and only then played a trump from dummy. East won heartA and gave his partner a trump promotion by leading a third diamond.
