If South leads a heart immediately then if West has the fourth diamond then he will rise with heartA and play the diamond to force dummy to ruff with heart9 - leaving dummy with the lone heartK. If West has started with heartAQx declarer will go down, so South pitches diamond9 to allow himself to ruff the fourth diamond in hand.
