After winning clubA you draw trumps in three rounds, cash three top hearts, all following, and ruff a heart - East has four. Presumably West has some club length for his double of 3club and as he also has three trumps and three hearts there is not much room for many spades. It's therefore better to play East for length in spades and as he could have both the vital missing honours you should lead small to the ace and back towards the queen. Unfortunately East is not playing ball and plays low.
