It's best to assume that the spade honours are split and 3NT will be defeated - if not then 4heart will probably be outscored anyway. To draw three rounds of trumps immediately might cause problems if one defender has heartJxxx and you fail to find diamondQ, so maybe it's just best to play for +420 with the outside chance of an overtrick...
