If you suspected that this might be a contrived hand then you'd be absolutely right - it comes from the 1951 Australian World Olympic par contest. The winning defence, as you probably guessed, is to under-ruff! Declarer is always going to finesse against your spadeQ so under-ruffing is the only way to score another trick, preventing East from establishing an extra winner in hearts or clubs.
