In fact North led spade2 as a suit preference signal, and after South had ruffed the spade a club was returned to North's ace. There was no second ruff from South so the trumps situation was clear to declarer, but because dummy's hearts could not be reduced by ruffing clubs without North ruffing in high, there was no trump end-play to avoid two trump losers. 4heart was light and South's judgement had been vindicated.

What if South had reopened with 4NT?.....
