Did you ask me whether East-West were playing strong tens? If so, then I would say no, and therefore North will be holding heartQ. With that in mind, and the fact that you cannot thwart either of the potential minor suit finesses, you should be contemplating winning heartA and switching to a spade. That is your first hurdle overcome, but the next follows very quickly.
