This calls for a partial elimination. You draw two rounds of trumps with spadeAK, cash your remaining clubs ending in dummy and lead a heart, intending to cover East's card. If East is either asleep or unable to win the second round of hearts then West will have to concede a ruff and discard after cashing his second heart trick. No problem!
