Obstruction or construction? I suppose it's all a matter of style and judgement - do you think that there is a possibility of game your way? If you take 1NT at face value, as North did at the table did, then it's probably best not to get too excited and raise quietly to 2spade. After that West put on some steam with 3NT, so North decided enough was enough and doubled, whereupon East reverted to 4heart.

The shock of the sight of dummy might be enough to send you into an apoplexy, but you must concentrate on beating 4heart. Declarer ruffs spadeK, draws trumps in four rounds (partner holding three) and plays a diamond to partner's eight and dummy's king. Your move.
