An inexperienced South won the opening lead with spadeA and led diamondQ. When East showed out, she rose with dummy's ace to play another trump. East won and continued spades, so declarer ruffed, drew the last trump and started the hearts. Fate was both unkind and just when they broke 4-1, for the slam was doomed when dummy had no late entry to the established hearts.

What South should have done was allow East to win diamondK at trick two. The spade return is ruffed and the hearts tested. When they prove to be 4-1 a heart is ruffed and two rounds of trumps drawn, ending in dummy. 12 tricks is the result.
