I'll use the words of my correspondent to explain:

However, I played 6spade off (!) after getting a diamond lead, but there were twelve easy tricks even if trumps had broken four-nil. Not the way I played it though - my mind must have been elsewhere. I decided not to ruff a diamond at trick two, but played two rounds of trumps and then started on clubs. Now when West ruffed the second club (the ace) I could no longer make the contract.

Whoops. It's hard not to feel sympathetic - we've all done something similar, but have a thought about the play in 7spade.
