The only option appears to play the 2spade opener for the ace of hearts without the Jack, so you play heartQ which holds and then a heart to the ten and ace. A spade is returned so you win with the ace, play a trump to South and after cashing the ace of clubs he is forced to give the entry to the table for two spade discards.
