The safest line is to cross to diamondA and lead a spade intending to finesse. If South wins and leads king and another heart for North to ruff at least declarer is safe from a trump promotion. In this case East over-ruffs, draws trumps and follows with king and another club to give himself the chance of the diamond finesse for his contract. At the table East exited with a heart at trick two and now the defence did find the trump promotion when North was able to ruff a heart with spade8.

In the other room South led a diamond against the same contract, so East had an easy +650.
