I hope that you will be way ahead of me, because it was your dozy correspondent who was asleep at the table. After ruffing the club declarer pitched clubQ on spadeA and led a trump from the dummy. Unlike me, you must be ready to rise with ace and shoot back a diamond. If partner does have the hoped for heartKx (what other hope is there?) then he will win the second round of trumps and give you a diamond ruff. Very straightforward really.
