Simply exit with spade3! East will have no hearts and a lead of either minor suit will give access to dummy and all its lovely winners. This line leads to twelve tricks and would seem to justify an earlier decision to go on.

Incidentally, if you do adopt the inferior line of ruffing a heart in dummy then East has to be very far-sighted. If over-ruffs dummy and exits with another trump then he must ensure that the one left in his hand is the five! Otherwise you still have the option of cashing clubAK and exiting with spade3 to put East on play.
