West must under-ruff when South ruffs the third round of diamonds! A club discard is fatal as you have seen, but so is a heart because declarer can now establish the hearts with a couple of ruffs. If West under-ruffs South's spade10 the defence must prevail. Although South is likely to see why West has under-ruffed it won't help. If declarer opts for a cross-ruff then East's spade5 becomes vital - South is unable to ruff all three of his club losers in dummy.
