Well you now have no option but assume the defenders to have certain key cards and play accordingly. Firstly, you must play for three heart tricks so you need East to have both queen and ten. You also require four club tricks but in addition you need two entries to dummy in order to finesse in hearts so you are forced to play West for the queen. When you have worked all this out the play becomes fairly straightforward.
