That diamond switch is a real pain, threatening to cut you off from dummy's juicy diamonds! It's not unreasonable to play West for heartKx(x) for this would allow you to clear all the trumps in three rounds to leave dummy's diamonds both accessible and uninterruptible. Any other possibilities? If the heartK was single in East's hand then a heart to the ace is the winning line - but it's very poor alternative.
