As you may well be stopping in 3NT, East must be denying four spades on this sequence, and if it was just showing a spade stop it will NOT have a heart stop as well or they would have just bid the appropriate number of No Trumps on the round before.

So...East is merely back checking to see if partner has a heart stop. (Remember the 3spade bid shows four spades but said nothing about hearts.)
