Not only should you rise with spadeA and unblock heartQ, but you should also cash clubAK too before playing spade9 from dummy! If you play low on the spade then East simply plays another, locking you in dummy. If you pass the first test and rise with spadeA, but fail to cash clubAK after making heartQ, then when you come off dummy with a spade East will rise and exit with a club!!! Again you will be stuck in dummy and again West's heart10 will be promoted.

The cashing of clubAK is our old friend the Dentist's Coup - you extract the teeth that are West's exit cards. Next time he'll know to have an anaesthetic when you're declarer. Either that, or make better leads.
