South wins diamondA, ruffs a diamond with spadeA and leads clubK. If East coves, fine, but South ruffs irrespective of East's card. As it happens East plays low, so South ruffs his last diamond with spadeK and leads spade10 intending to overtake. After forcing out spadeQ and ruffing the diamond return, declarer draws the remaining trumps and cashed the hearts. All in all, declarer needs a three-two trump break in both major suits, with the caveat that South can cope when East has four hearts.
