. . . but the winning defence is to switch to a heart. Assuming that South's hand is consistent with the "not much outside" definition, this can only be wrong when East has clubQJ4 and South eight diamonds. It’s best if West switches to heart10 to nullify the threat of South's heart9, otherwise declarer can duck the heart to East. In this latter case East must cleverly switch to a diamond, forcing declarer to run the diamonds and subject dummy to a three suit squeeze.
