East forgot that he and his partner had agreed to play splinters in this position, so his partner was showing a singleton spade. Rather than sign off with 4heart (look at those wasted spade values) or cue-bid with 4diamond, East raised to 4spade thinking that 3spade showed a suit. Whether West realised that a wheel had come off or that he interpreted 4spade as showing the ace, we may not know, but the ambiguity was cleared by his jump to 6heart.
