Look at East's dilemma as the spade is played from dummy. If East throws a diamond declarer ruffs with the king and leads another club, won by East. What does East do? To lead a trump means that dummy will be high, but to lead anything else allows South to cross-ruff the last two tricks.
It's similar if East ditches a club on spade7, for now South ruffs with the jack and leads a club. East can give South the last two tricks by playing a trump or alternatively let heartK and heartQ make separately.
Finally, if East decides to ruff the spade then South makes all her trumps separately without any complications.
