Ask yourself this: with a doubleton in dummy would you have covered heartQ with South's heart holding? Probably not - it could easily be wrong if declarer has heartAJ10xx(x). What about covering with heartKx or heartKxx? That's much more likely! The subtle inference that South had not covered was not lost on Duboin - the hearts were not breaking.
