East had petered (high-low) in hearts and, sensing the suit was 6-2, South led heart8 off the table with the intention of throwing diamond7 on it. This elegant loser-on-loser would have end-played West, but East was alive to the danger and promptly ruffed! Declarer was still not dead, for now he had a complete count. Over-ruffing East's spade7, he simply played ace, king and another club. Either clubQ would drop from East or West would be left on lead after winning with the queen, forced to concede the tenth trick to clubJ or via a ruff and discard. Well defended and well played!
