. . . declarer leads heart9. Look at the end position. When dummy plays low on East's queen what is West to do? He has to decide whether to allow East to be end-played or take on the same sacrificial role himself!

Of course it was East's fault all along. He should have foreseen this ending at trick nine - assuming that North started with only two hearts East should have retained a vital low heart and unblocked the ten. When diamondQ follows next East must continue unblocking with the queen - it cannot cost. Study the diagram again to confirm - if North's "missing" heart is the jack then the contract is unbeatable anyway. The full hand .
