Of course it was East's fault all along. He should have foreseen this ending at trick nine - assuming that North started with only two hearts East should have retained a vital low heart and unblocked the ten. When Q follows next East must continue unblocking with the queen - it cannot cost. Study the diagram again to confirm - if North's "missing" heart is the jack then the contract is unbeatable anyway.
