This was the five card ending: when East played clubAQ from dummy what was he supposed to throw from hand? To retain diamondQ he threw a heart, but now South switches to heartJ and North takes the rest of the tricks! This was a true suicide squeeze.

Declarer would have been much better advised to cash at least three spades before exiting with club8 - the contract is then unbeatable. East can also get home by exiting with heartQ in the shown end position.. and, of course, had West leapt to 3NT originally then North would have been hard pushed to resist a heart lead.
