With a club holding like North's it's almost certain that East will be going after a red suit, and if a heart is led from dummy North should be going up with the ace like a flash, rather than playing low in automatic pilot mode. The logic in this is that it will retain any heart entry to South's potential spade winners. But it didn't go like that.
