The problem with South's 3NT is that it could be made on anything within a very wide range - let's say 15 to 22. If North's hand is such that he feels he can underwrite 4NT, then it's a nice treatment to use all bids the four level as Baron, with 4NT available as the fall-back. So, if North judges that 4NT is likely to be OK, then he can bid 4club to show a club suit and a very mild slam try.

You can imagine that South would not now stop short of 6club. How would you play on heart3 lead?
