If partner's heart2 is to be believed then he has five, and in that case declarer is very likely to be 2-3-5-3. He knows that if declarer has diamondAQJ then the contract is cold, so he has thrown diamonds to clarify the situation for you. You can count declarer's high cards as heartK, diamondAQ and clubAQJ, so it's clear to retain at least two hearts and all four diamonds. This means it would also be a good idea to unblock one of your spade honours, for you would not want to be left on lead and forced to open up the hearts.
