South made a classic error by leading another heart from dummy. All was that required was for her to enter hand with diamondK and lead a trump towards dummy. This means that when West flies in with the ace, declarer still retains her queen as insurance against a trump promotion.

At the table South's failure to do so was fatal. West won South's heartQ with his ace and led a spade his partner's ace. West ditched spadeQ on clubQ and a third round of spades promoted heart9 as a winner - West's bold gamble had come off.
