The best defence to beat 4 is a heart lead, ducked to the king, and a spade switch. When East gets in with
Q he reverts to hearts and dummy's only entry has gone before declarer can organise any end-play on West. An initial club lead is OK too I suspect, but East needs to find an heart switch at trick two
