East did not find the very best defence when he switched to a heart, despite West's signals, so declarer played low and West won the king, North unblocking the queen. Seeing that he was going to be endplayed if he defended passively, West returned a diamond - playing East for diamond109xx. This was not a success, and suffered more misery when handing North an unexpected overtrick.

The best defence to beat 4spade is a heart lead, ducked to the king, and a spade switch. When East gets in with spadeQ he reverts to hearts and dummy's only entry has gone before declarer can organise any end-play on West. An initial club lead is OK too I suspect, but East needs to find an heart switch at trick two
