I think a lot depends on North's ambitions and current estimate of the other North-South pairs and his side's standing. If he desperately needs a big score, he should win and take a trump finesse. When that holds he can cash heartAK and play on diamonds, making eleven or twelve tricks depending on whether heartQ comes down. If he considers that an average board is enough, he will note that on a spade lead 3NT is doomed and that 5diamond will not outscore 4heart. In that case he might decide to cash heartA,clubK and play on diamonds immediately. This might lead to a defensive diamond ruff, but 4heart should be reasonably safe, and plus 620 will yield a fair score.
