After South has revealed that he holds 10 or probably 11 minor suit cards, East would only be wrong to cover if South started with spadeJ10heart-diamondAKx and a club suit as good as clubAK10xxxxx. If South had not cashed heartA East might have received the impression that the ONLY possible trick he can win is clubQ. After all, the hearts will be coming in for at least three tricks because declarer "must" have one - he hasn't cashed heartA. This surely means that East would be less likely to cover club9. When that holds then heartA is cashed and the diamond finesse follows.
