At the table East switched to a small trump, which was not good enough. Declarer won with spadeQ, played heartA discarding his last club, spade10 which held, and another which East won with the ace. He now exited with a heart so declarer ruffed, drew East's last trump and claimed. Where did East go wrong?

By not simply continuing with clubQ! Partner's helpful count has told you that North has another club so declarer must ruff the club in dummy. After cashing spadeQ10, which of course you duck, declarer can only get home if he holds heartK as an additional entry. No doubt he will see that the hand goes out of control if he ruffs a heart in hand so he will attempt to return via diamondJ. You will know what to do with that.
