He shouldn't ruff the club! Cash the spades by overtaking the queen with the king and continue with spade winners, then when East ruffs the spade10 with heart6 dummy still has a late entry to lead the last spade. East can ruff this with heartA, but it beats the air. Easts failed to ruff the fourth spade because - "I assumed declarer was sane".

Mind you, I cannot see that East had a monopoly in the sanity stakes - it can hardly cost to ruff the fourth spade with heart6.
