Notice that even if the trumps were 2-2 and East had ruffed a heart at any stage, South's diamond would be discarded and there would still be only two trump losers in total. After the initial lead, the only likely distribution that would have defeated declarer would have been a doubleton heart with West, when he could over-ruff South and cash a diamond.
